Ran into a little problem today.
I have been running SCS 10.9.4 and upgraded to 10.9.6. When I ran the first cue in the show which is a Playlist cue, my FOH and ROH and Ouside speakers were out of sync.
I have the following devices set up:
FOH Stereo USB1 Chs 1,2
On Stage R Mono USB2 Ch 2
On Stage L Mono USB2 Ch 1
ROH Mono Internal Sound card ch3
Outside Mono Internal sound card ch4
Bug or a setting I am missing?

I wouldn't think it's a bug. Unfortunately it is possible for speakers to be slightly out of sync, but this should be very rare with the changes in 10.9.
What sound interfaces are you using? Using multiple sound interfaces is very likely to give you syncing problems as different drivers can have different latencies. From the info you've supplied it looks like you're using 3 different interfaces: USB1, USB2 and internal sound card. It would be better to have a single USB interface which supports at least 6 outputs, preferably with full WDM/DirectSound support. That rules out anything from M-Audio, but a Roland Octa-Capture will do the job if the budget will cover it.

Sorry I let this go so long.
The show finally closed and I have been enjoying having my weekends back!
Rolling back to 10.9.4 corrected the problem.
The delay was very noticable almost a second or two.
Any settings I can check or change to help the problem?
