Just had a very bad day with 11.4.2.3
I edited a new show, starting, as usual, with a basic show file I use as a template. This has our standard 8 output ASIO card as its device and a couple of pre-show announcements. A good start!
I added the few cues I needed (it's a simple show). This worked fine. Then I closed the program. So far, so good.
On restarting SCS, cues wouldn't play. I opened the editor and tried to select a cue. SCS said I had Device Map errors, so I went to the Devices tab in Production Properties. The first 4 columns looked fine but the "Outputs" column was empty. I can't remember if it let me fix this, but I think it did not. "Apply Device Changes" was always highlighted, but clicking it had no effect. I couldn't go anywhere else in the program because I had unapplied device changes! Clicking "Undo Device Changes", greyed out "Apply Device Changes" and took me back to the non-working state I started with. I never got to a position where I could generate a Diagnostic File, I'm sorry to say.
Last chance, I restarted the PC, no improvement.
I looked to see what other releases I had on the computer and re-installed 11.3.5
The same cue file now works and SCS has no problem with the soundcard.
I'm sorry the information is rather limited Mike, but hope it gives you some clues.

I recently had a report from another user that the 'Apply Device Changes' button was always enabled. After running a trace it was found that the reason was that the Video Audio Device had not been selected. There will be a change in 11.5 that will display a message giving the reason(s) that 'Apply Device Changes' is still enabled after clicking that button.
Although this may well also be the problem in your case, I will email you details of a tracing version to run to try to find out why your device map is apparently reporting errors.

Thanks Mike. I have been unable to reproduce the fault today, with 11.4.2.3 or your tracing version.
Hopefully the diagnostic files will still give you a clue.

I had similar experience yesterday, fault was on a "silent" install of the driver update for internal sound card (usually windows default) which changed name of the device and it was unrecognizable to SCS.
Usually ( as Mike described ) default windows device is default in SCS too, for Video Audio Device.
It took me a while to recognize the problem by checking drop-down list of devices in Video Audio Device setup.
SCS "remember" previously assigned device in the device map, and if it is not there it will make a small havoc, so promise of error explanation in v11.5 is more than welcome!
